Intake summary refers to a report written by the clinician that provides a detailed snapshot of all the basic information gathered from the client’s initial contact . Intake summary typically includes identifying information, presenting concerns, background history, problem and counseling history, mental status examination, and goals for counseling and course of counseling to date.
